چکیده

Achalasia of the oesophagus is characterized by the absence of peristalsis, failure of the inferior oesophageal sphincter to relax after swallowing (Harris, 1966), and a positive mecholyl test (Kramer and Ingelfinger, 1951). This combination offeatures is regarded as specific for this disease. Nevertheless, in the literature a number of cases have been designated achalasia despite the presence of some peristalsis or inferior sphincter relaxation (Vantrappen, van Goidsenhoven, Verbeke, van den Berghe, and Vandenbroucke, 1963) or even a negative mecholyl test (Hightower, Olsen, and Moersch, 1954). Therefore, absolute diagnostic criteria for the disease seem to be incompletely defined. We have studied six patients with severe oesophageal motor abnormalities referred to us with the clinical diagnosis of achalasia. The oesophageal motility findings in these patients were, however, not characteristic for achalasia; moreover, they were

: the management of achalasia – a comparison of different treatment modalities 1

Background Achalasia is an uncommon primary oesophageal motor disorder with an unknown aetiology. Therapeutic options for achalasia are aimed at decreasing the lower oesophageal sphincter pressure, improving the oesophageal empting, and most importantly, relieving the symptoms of achalasia. Modalities for treatment include pharmacologic, endoscopic, pneumatic dilatation and surgical. The decisi...

Achalasia with massive oesophageal dilation causing tracheomalacia and asthma symptoms

Achalasia is an uncommon oesophageal motor disorder characterized by failure of relaxation of the lower oesophageal sphincter and muscle hypertrophy, resulting in a loss of peristalsis and a dilated oesophagus. Gastrointestinal symptoms are invariably present in all cases of achalasia observed in adults. We report a case of a 34 year-old female patient with long standing history of asthma-like ...

Non-achalasic motor disorders of the oesophagus.

Motor abnormalities of the oesophagus are characterised by a chronic impairment of the neuromuscular structures that co-ordinate oesophageal function. The best-defined entity is achalasia, which is discussed in a separate chapter. Other motor disorders with clinical relevance include diffuse oesophageal spasm, oesophageal dysmotility associated with scleroderma, and ineffective oesophageal moti...

Oesophageal and gastric motor activity in patients with bulimia nervosa.

Previous studies showed that symptoms of oesophageal motor disorders can be misinterpreted as indicating anorexia nervosa and that in primary anorexia nervosa gastric motility is frequently impaired. We investigated in 32 women with bulimia nervosa whether symptoms of oesophageal motor disorders could be obscured by or be mistaken as forming part of bulimic behaviour, and whether impaired gastr...

Anomalies of peristalsis in idiopathic diffuse oesophageal spasm.

Oesophageal manometry revealed 'interrupted peristalsis'--proximal sequential contraction with simultaneous pressure increases in the mid-oesophagus and sequential contraction distally--and abnormally slow distal propagation of peristalsis in seven of a consecutive series of 12 patients with idiopathic diffuse oesophageal spasm (IDOS). These two abnormalities occurred together in six patients. ...
